How to record screen activity and voice.?

I need to develop a video tutorial for using a Microsoft Office program, something like the videos that Total Training and Lynda.com do, but a bit simpler. For that purpose I need to be able to:
1. record what I am doing on the screen.
2. record my voice, as I need to give some explanation.
I have never done that before and I don't know which software to use, what file format to choose. So I would be really grateful if you can give me any tips. Thank you
